Privacy policy
This policy explains what information this website collects from visitors, how it's used, and the choices you have. I collect as little as I can, I never sell it, and I don't use advertising trackers.
Already a client? Your health information is protected by HIPAA and explained in my Notice of Privacy Practices. If the two documents ever differ about your health information, the Notice of Privacy Practices applies.
Who this policy covers
This policy covers pathtorisetherapy.com, run by Path to Rise Therapy LLC (Yvette Ramos, LCSW). It applies to anyone who visits the site, calls, texts or emails the practice, or requests a free consultation.
What I collect
Information you choose to share
- Your name, email, phone number and preferred times when you request a consultation.
- Anything you write in a consultation request, email, text or voicemail.
- Your state of residence and insurance carrier, if you share them to check fit or coverage.
Information collected automatically
- Basic technical information that every website host records to deliver and protect the site, such as your IP address, browser type, device type, the pages you visit, and the date and time of your visit.
- Cookies the host needs to run the site, such as security and load-balancing cookies.
This site does not use advertising cookies, social media pixels, or tracking tools that follow you across other websites.
Please keep health details out of email and forms
Regular email, text messages and website forms are not a fully secure way to share health information. When you reach out, a short note about what you're looking for is plenty. We'll talk through the rest on the consultation call, and clients share details through my secure client portal.
How I use your information
- To reply to you and schedule a consultation.
- To see whether we're a good fit, and to refer you elsewhere if we're not.
- To provide and bill for services if you become a client (covered by the Notice of Privacy Practices).
- To keep the site working, secure and easy to use.
- To meet legal, licensing and professional obligations.
I don't sell, rent or trade your information, and I don't use it for advertising.
Who I share it with
I share information only with service providers that help run the practice, and only what they need to do that work:
| Provider | What it does |
|---|---|
| SimplePractice | Consultation requests, scheduling, client portal, records, billing and telehealth. SimplePractice is HIPAA-compliant and signs a business associate agreement with the practice. |
| Mentaya | For clients who use out-of-network benefits: estimates what your plan will reimburse and submits your superbills to your insurer. |
| Website host | Delivers this site and keeps basic server logs for security. |
| Email and phone providers | Deliver the messages you send and that I send back. |
I may also share information when the law requires it, to prevent serious harm, or to protect the rights and safety of you or others.
Scheduling through SimplePractice
The "Book a free consult" buttons take you to my SimplePractice page (yvetteramos.clientsecure.me). Information you enter there is collected and protected by SimplePractice under its own privacy policy and its agreement with my practice.
Links to other sites
The site links to other websites, such as the 988 Lifeline and SimplePractice. Their privacy practices are their own, so please read their policies.
How long I keep it
If you contact me and don't become a client, I keep your message only as long as needed to respond and follow up, then delete it. If you become a client, your records are kept for as long as state law and professional rules require.
How I protect it
I use services that encrypt information, protect accounts with strong sign-in security, and limit access to people who need it to do their work. No method of sending or storing information online is perfectly secure, so I can't promise absolute security.
Your choices
- Ask what information I have from your website contact, and ask me to correct or delete it.
- Ask me to stop contacting you.
- Block or delete cookies in your browser settings. The site still works without them.
To make a request, email or call using the contact details below. I'll respond within 30 days. Depending on where you live, you may have more rights under state law, and I'll honor them.
Adults only
This site and my services are for adults 18 and older. I don't knowingly collect information from children under 13. If you think a child has sent me information, contact me and I'll delete it.
Changes to this policy
If I change this policy, I'll post the new version here and update the date at the top.
